The safety and well-being of our Scouts, volunteers, and families are a priority for Sequoia Council. This page brings together incident reporting forms, online reporting tools, insurance claim resources, and crisis communication guidance to help leaders respond promptly and connect with the appropriate support.

Youth Safety & Membership Concerns (Riskonnect)

Use the Riskonnect online reporting tool to report a concern or incident. Select the category that best matches your report:

  • Membership Infraction: Select this for Youth Protection violations, allegations of abuse, self-harm, or behavioral concerns that put a Scout’s safety at risk. This also includes concerns about a registered adult or youth member’s continued registration with Scouting America.

  • Near Miss: Select this for an unplanned event that caused no injury, illness, or property damage but had the potential to do so. Do not use this category for COVID reporting, Youth Protection concerns, or behavioral issues.

  • General Liability: Select this for allegations of bodily injury, illness, or property damage, including employment matters. Use this category for COVID reporting.


Injury, Illness & Damage Reporting

Report an injury, illness, or property damage connected to a Scouting meeting, outing, event, or related travel.

Download and complete the fillable form, then submit it to the Sequoia Council office in person or by email to Membership@SequoiaCouncil.org

Insurance Claims

Download the Accident and Sickness Insurance Claim Form for a covered injury or illness related to a Scouting activity. Complete the form as fully as possible, then return it to the Sequoia Council office in person or by email to Membership@SequoiaCouncil.org.
